Please read all below information before you fill in a registration form.

I, THE PARENT/GUARDIAN OF THE ABOVE-NAMED PERSON, GIVE MY APPROVAL TO HIS/HER PARTICIPATION  IN ALL LITTLE LEAGUE ACTIVITIES. MY SON/DAUGHTER IS NOT SUFFERING FROM ANY HEALTH PROBLEMS THAT MAY BE AGGRAVATED BY HIS/HER PARTICIPATION IN FOOTBALL MATCHES OR TRAINING.                                                                                                                               

I DO NOT HOLD THE LEAGUE OR THE LEAGUE ORGANISERS OR THE LITTLE LEAGUE FOOTBALL NATIONAL EXECUTIVE RESPONSIBLE FOR ANY CLAIM ARISING OUT OF ANY  INJURY SUSTAINED BY MY CHILD EXCEPT TO THE AMOUNT COVERED BY THE PUBLIC LIABILITY INSURANCE POLICY ARRANGED BY THE SURREY FOOTBALL ASSOCIATION. CHILDREN LEFT UNATTENDED ON MATCH DAYS ARE LEFT AT PARENT'S / GUARDIAN'S OWN RISK.                                                                                                         

CARSHALTON LITTLE LEAGUE HAS PUBLIC  LIABILITY INSURANCE BUT DOES NOT HAVE PERSONAL INSURANCE FOR EVERY PLAYER IF YOU WISH TO TAKE OUT YOUR OWN PERSONAL INSURANCE TO COVER YOUR CHILD AT TRIALS/TRAINING/MATCHES AND THE NATIONAL TOURNAMENTS YOU ARE WELCOME TO DO SO.                                                                                                                       

THE PARENT/GUARDIAN OF ALL BOYS/GIRLS SELECTED TO PLAY IN SENIOR/INTERMEDIATE/JUNIOR & BANTAMS TEAMS ARE EXPECTED TO PAY A REGISTRATION FEE EACH FOOTBALL SEASON. THE COST OF REGISTRATION IS £50 ONCE THIS IS PAID YOUR CHILD IS GUARANTEED A PLACE IN OUR LEAGUE   . PLEASE NOTE IF YOUR BOY/GIRL LEAVES THE LEAGUE AFTER YOU HAVE PAID THE REGISTRATION FEE THERE IS NO REFUND.                                                                                                                               

THIS FEE COVERS UP TO 26 GAMES PER SEASON FOOTBALL SHIRT, SHORTS AND SOCKS ARE SUPPLIED BY LEAGUE PLUS A TROPHY FOR EVERYONE AT THE END OF THE SEASON. THIS IS A ONE OFF FEE BUT TOWARDS THE END OF THE SEASON WE DO A SPONSORED PENALTY SHOOTOUT COMPETITION AND WOULD EXPECT EVERY PLAYER TO PARTICIPATE AS THIS IS OUR ONLY FUNDRAISER ALL SEASON.                                                                                                                             

NO CHILD WILL BE ALLOWED TO PLAY ABOVE HIS AGE GROUP BUT A CHILD MAY BE ALLOWED TO STAY DOWN IF IT IS IN THE BENEFIT OF THE CHILD AGREED BY THE COMMITTEE.                                                                                                                      

Please note that due to legislation changes affecting Data Protection, we as a league must inform parents/guardians completing, this form that it, and the personal data contained on it, will be stored safely and securely, and in line with the League Data Protection Policy Personal data will only be used for registering players to Carshalton Little League. This form and the personal data on it will be retained for the period of your child’s stay in the League, after which it will be destroyed along with any database used for the production of the registration. A parent/guardian signing this document should be aware that, in doing so, they are giving consent for their son/daughter's data being used for Registration to the League. The league understands that consent can be withdrawn by the person giving it at any time and, once notified, will immediately destroy the relevant data, and cancel the player's registration.
